As a privately owned practice, we value lasting relationships and take time to listen, understand your concerns, and partner with you in your care. Guiselle Berry, NP, founder of Revive Family Care & Wellness, is an experienced, certified family nurse practitioner who incorporates a functional medicine approach to supporting patient well-being. With the addition of three providers to our team, we are expanding access to personalized care for the communities we serve.

We welcome open conversations about vaccines and your family’s questions. We do not dismiss patients from our practice based on vaccination status or vaccination decisions.

Our team diagnoses and treats a wide range of health concerns, including diabetes, high blood pressure, cardiovascular disease, emotional and mental health concerns, menopause symptoms, low testosterone in men, and common illnesses such as colds, strep throat, and stomach flu.

Our services include sick visits, well-child visits, hormone replacement therapy, and IV vitamin and mineral therapy. We also offer medical weight management, with treatment options that may include semaglutide or tirzepatide when clinically appropriate.

Aesthetic services are offered through our sister company, Divine Aesthetics Center, located within our practice. Services include radiofrequency (RF) microneedling with Sylfirm X and topical exosome application. Injectable treatments include Botox®, Jeuveau®, and Letybo® for expression lines, along with Evolysse® Form and Evolysse® Smooth hyaluronic acid facial fillers.
